There are several potential causes for your recurrent chest and shoulder pain. One of them is a post surgical inflammatory response that causes irritation ofthe sac lining the heart, the pericardium. It is so distinct it actually carries an eponym, "Dressler's syndrome." I would leave it to your physician to make the diagnosis and prescribe appropriate treatment, as there are other possibilities as well.
