I think the rationale for putting folks on warfarin (coumadin) for a period post op is that the atria don't start beating effectively for a while even though they show a return to sinus rhythm electrically. The signal is there but the muscle can't respond. This also happens for a period of time after cardioversion. So, for a time, the risk for atrial clot formation post op may be the same as it was pre-op. That's the pro coumadin argument. The anti coumadin argument is that in folks under age 65 with lone af (no hypertension, no coronary artery disease, valvular heart disease or cardiomyopathy), the risk for atrial clot formation was pretty small in the first place. In addition, most atrial clots form in the left atrial appendage , so some people would argue that amputating the left atrial appendage alone (done routinely as part of the maze) decreases the risk of clot, even if you remain in AF.
