I have standard blue cross blue shield of Missouri. I referred myself to Dr. Cox while he was at Georgetown. They initiated contact with the insurance company and I had no problems with coverage. My out of pocket expense was $25. The negotiated price was ~$25,000. I don't know the "sticker price" for the whole thing, but the negotiated costs to third party payers are typically less than 50%, so I am sure it was in the same ballpark - $50-60K
