Everyone's insurance is different. My insurance card, as well as their web page will tell me who is, and is not covered.
The facility chosen should help you also. Just be sure to ask if they are covering both surgery and hospital stay. I got permission to have surgery at Mayo one time, and found that it only covered surgery, and the hospital stay the day of surgery, and not the following four days!
