I also fought my insurance company regarding this matter. Dr. Schweikert (Dr. N's colleague at CC)wrote directly to my insurance company. I will search my files and see if I can find the letter he wrote ... I know his letter was a big influnce in my case. Also, have you spoken to you State Insurance Commissioner regarding this matter?

Please feel free to email me personally if there is anything I can share to help ... in the meantime, I will search for that letter from my doc at CC.
