Sorry, Bill, I've been out enjoying my new found post maze energy levels so I haven't replied to your request.
My insurance company was Blue Shield and they covered my procedure. My advice to anyone who is worried about whether their insurance will cover their procedure is to let the hospital worry about it. They are used to dealing with almost all of these companies and if anyone can get an approval, they can. It's money they want and they are pretty good at getting it.
I went to Cleveland and left it in their hands. It worked out well for me.
